Prolonged services coverage criteria

Covered when documentation validates extended time and the specific documentation and code‑specific rules below are met:

ALL of the following

Supporting documentation must be filed with the claim at the time of submission; claims for prolonged services will automatically suspend for individual consideration review and BCBSRI will not request documentation later; claims submitted without documentation will be denied as documentation required.

ALL of the following

Documentation must include the start time and end time of the visit or the total time spent, including direct contact and non‑face‑to‑face time (excluding telephone encounters).
See requirements for component‑level timing when multiple face‑to‑face and non‑face‑to‑face segments occur during the date of service.
Documentation must reflect the reporting practitioner's time and show that the provider personally furnished the prolonged service time; services provided by another provider or support staff will not be considered for reimbursement.
If time is used to select the E/M level, the medical record must indicate when counseling was the dominant service provided.

ALL of the following

G2212 (Medicare Advantage): May be reported when the maximum time for the level 5 office/outpatient E/M visit is exceeded by at least 15 minutes; do not report G2212 for any time unit less than 15 minutes; do not report G2212 on the same date of service as 99354, 99355, 99358, 99359, 99415, or 99416.
99417 (Commercial): BCBSRI will not follow AMA guidance that allows 99417 to be reported within the first 15 minutes of extended time for Commercial members; reimbursement consideration differs from AMA guidance.
99354–99357 (prolonged outpatient/inpatient codes): 99354 and related codes should not be reported separately if the total duration is less than 30 minutes on a given date; 99355 and 99357 are the additional‑time units (each additional 30 minutes).

ALL of the following

Prolonged add‑on codes are time‑based and require that the primary E/M service have a typical or specified time published in CPT.
99358 and 99359 may be used in the same session as an E/M service except for office/other outpatient services (99202‑99205 and 99212‑99215); they may also be used on dates different from the primary service. When filed, supporting documentation must include records for both the prolonged service and the related primary service.

ALL of the following

99415 and 99416 are covered but not separately reimbursed; administration of medication requiring observation should be filed with 99415/99416 and it is incorrect to file prolonged physician services for time spent by clinical staff observing a patient.

ALL of the following

Time performed solely by office staff without physician/qualified practitioner time.
Certain hospital chart review without provider face‑to‑face time, preventive medicine, emergency medicine, critical care, neonatal intensive care, anticoagulation services, care plan oversight, overlapping management services, team conferences, telephone encounters, and multiple use of the same prolonged code on a single date when not supported by time documentation.

99417 and G2212 general handlingClaims filed for professional outpatient prolonged services (both 99417 and G2212) will be considered per CMS Prolonged Office/Outpatient E/M Reporting Times; time of the reporting practitioner is used to select the office/outpatient E/M visit level.
