BCBSRI follows CMS NCCI PTP guidance: append modifier 59 or an appropriate X{EPSU} modifier to the Column 2 code when documentation supports that the services are separate and distinct; use X{EPSU} modifiers when they more specifically describe the circumstance and do not append both 59 and an X modifier on the same line.
