Documentation must support separation by one or more of the following: separate session/patient encounter, separate procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries).
Do not append modifier 59 when a more descriptive X{EPSU} modifier is available; do not report modifier 59 and an X modifier on the same line.

BCBSRI follows CMS NCCI PTP guidance: when two codes in a Column1/Column2 edit pair are reported for the same patient on the same date, Column1 is eligible for payment and Column2 is denied unless documentation supports that the services are separate and distinct and modifier 59 or an appropriate X{EPSU} modifier is appended to the Column2 code. Use the more specific X{EPSU} modifiers when they better describe the circumstance and do not append both modifier 59 and an X modifier on the same line.

If both codes represent separate and distinct procedures/services, append modifier 59 or X{EPSU} to the Column2 code to be eligible for payment.
CPT guidance: do not use modifier 59 when a more descriptive X{EPSU} modifier is available; do not report modifier 59 and an X modifier on the same line.

Distinct Procedural Service (modifier 59)

Definition (modifier 59)Distinct Procedural Service — used to identify procedures/services (other than E/M) that are not normally reported together but are appropriate under the circumstances; documentation must support separation by session/encounter, procedure/surgery, site/organ system, separate incision/excision, separate lesion, or separate injury/area of injury.

E/M servicesModifier 59 should not be appended to an E/M service; to report a separate and distinct E/M service with a non‑E/M service on the same date, use modifier 25.
