Coverage Criteria for Bamlanivimab
Coverage criteria
Covered when ALL of the following criteria are met (BlueCHiP for Medicare and Commercial):
ALL of the following
- positive COVID-19 test results
- patient over 12 years of age
- at high risk for progressing to severe COVID-19 and/or hospitalization
- treatment given within 10 to 12 days of symptom onset
- patient not hospitalized
