Autologous PRP is covered for the treatment of chronic non-healing diabetic wounds when filed with a covered diagnosis and prepared by FDA-cleared devices for exuding cutaneous wounds.

Treatment is autologous platelet-rich plasma (PRP)
Wound is a chronic non-healing diabetic wound/ulcer
PRP is prepared by devices whose FDA-cleared indications include the management of exuding cutaneous wounds (such as diabetic ulcers)

Medicare Advantage - Other Chronic Wounds

Coverage determination for other chronic non-healing wounds.

Background: PRP contains platelet‑derived growth factors (eg, PDGF, transforming growth factors, VEGF) thought to aid wound healing; autologous platelet concentrate suspended in plasma is prepared from centrifuged autologous blood and, when activated (eg, thrombin and calcium chloride), releases growth factors and can form a platelet gel for application to wounds. Evidence summary: systematic reviews, meta‑analyses, and multiple small controlled trials have evaluated PRP for chronic wounds with mixed results. For diabetic lower‑extremity ulcers, some meta‑analyses and trials demonstrated improved complete wound closure and healing time, but studies have moderate‑to‑high risk of bias and imprecision limiting conclusions about other outcomes (recurrence, infection, amputation, quality of life). For venous and pressure ulcers and for acute surgical/traumatic wounds, results are mixed or show no clear benefit on important outcomes, and overall the evidence is insufficient to determine net health benefit for many wound types.
