Coverage Criteria

Medicare Advantage — Chronic non-healing diabetic wounds

Covered when ALL of the following are met

Medicare Advantage coverage for autologous PRP: Autologous platelet-rich plasma (PRP) is covered for the treatment of chronic non-healing diabetic wounds when filed with a covered diagnosis and when prepared by devices with FDA-cleared indications that include management of exuding cutaneous wounds (such as diabetic ulcers).20 weeks (CMS national coverage decision)

Coverage beyond 20 weeks and for other chronic non-healing wounds will be determined by local Medicare Administrative Contractors (MACs).

Commercial Products — All other indications

Not covered / not medically necessary

Commercial products and other indications: For Commercial Products, use of platelet-rich plasma (autologous blood-derived preparations) is considered not medically necessary for the treatment of acute or chronic wounds, including surgical wounds and nonhealing ulcers, because the evidence is insufficient to determine effects on health outcomes. Use of HCPCS G0460 for non-diabetic chronic wounds/ulcers is not covered.

Providers should verify member benefits; claims for non-covered services may be denied per the member's contract.

For Commercial products, the use of autologous platelet-rich plasma (PRP) for the treatment of acute or chronic wounds — including surgical wounds and nonhealing ulcers — is considered not medically necessary. The evidence is insufficient to determine that PRP improves net health outcomes for these indications, and HCPCS G0460 (autologous PRP for non‑diabetic chronic wounds/ulcers) is not covered for non‑diabetic chronic wounds/ulcers.

Coverage and payment for services described in this policy are subject to the member’s subscriber agreement, member certificate, or employer agreement. For services that are non‑covered benefits, providers must obtain written member agreement before charging the member for those services.
