Clinical Context

Electrical stimulation (ES) and electromagnetic therapy have been evaluated as adjuncts to standard wound care for chronic wounds. ES involves applying electrical current through electrodes placed on the skin and includes modalities such as low-intensity direct current (LIDC), high-voltage pulsed current (HVPC), alternating current (AC), and transcutaneous electrical nerve stimulation (TENS). Electromagnetic therapy applies electromagnetic fields rather than direct electrical current and is a related but distinct treatment approach. The clinical focus of the literature and this policy is on chronic wounds — wounds that persist for longer than 1 month — and specifically on the wound types most frequently studied: pressure ulcers, venous ulcers, arterial ulcers, and diabetic ulcers. Conventional therapy for these wounds includes local wound care (debridement, cleansing, moist dressings), systemic measures (infection control, nutrition optimization), and off-loading or not bearing weight as appropriate.

Available evidence consists of small randomized controlled trials and systematic reviews. Some studies report improvements in intermediate outcomes (for example, reductions in wound size or increased velocity of healing), but there is insufficient high-quality RCT evidence

Currently, no electrical stimulation or electromagnetic therapy devices are FDA-approved specifically for wound healing; several devices cleared for other indications are used off-label for wound therapy. As reflected in the coding guidance, a trial duration of 30 days of conventional care is referenced in code descriptors, but this policy considers these therapies not medically necessary due to insufficient evidence of clinical benefit.
