Coverage Criteria

Initial PTNS Therapy (Medicare Advantage Plans and Commercial Products)

Percutaneous tibial nerve stimulation (PTNS) is considered reasonable and necessary when ALL of the following are met:

Initial PTNS criteria: An evaluation by an appropriate specialist (usually a urologist or urogynecologist) has been performed and the specialist has determined the patient is a candidate for PTNS.

Behavioral therapy: Medical record documents the member has been compliant with and failed a trial of symptom-appropriate behavioral therapy of sufficient length to evaluate potential efficacy.

Medication trials: Medical record documents the member has been compliant with and has failed or been unable to tolerate a trial of at least two appropriate medications administered for four (4) to eight (8) weeks.

Voiding diary: The voiding diary shows continued findings of overactive bladder syndrome (OBS).

Patient willingness and compliance: Beneficiary has documented willingness to attend in-office treatment sessions, to comply with behavioral therapies, and to continue to keep voiding diaries including documentation of behavioral therapy compliance.

Treatment regimen: Treatment will consist of an initial course of one 30-minute session per week for 12 weeks.

Maintenance/Retreatment

Treatments for relapse are allowed only when the following is met:

Initial response required: Patient must have achieved a greater than 50% decrease in overactive bladder symptoms with the initial treatment.

Subcutaneous/Implantable Tibial Nerve Stimulation

Coverage stance for implantable tibial neurostimulators (e.g., eCoin):

Not covered: Subcutaneous tibial nerve stimulation delivered by an implantable peripheral neurostimulator system is not covered/not medically necessary for all indications including non-neurogenic urinary dysfunction such as overactive bladder due to insufficient evidence of net health outcome improvement.

CPT code 0816T (open insertion or replacement of integrated subcutaneous posterior tibial neurostimulation system) is not covered for Medicare Advantage Plans and is not medically necessary for Commercial Products.

Member benefits and eligibility are determined by the member's subscriber agreement or employer agreement, which supersede this medical policy. For member-specific benefits and eligibility, contact the provider call center. If services are determined to be not medically necessary or to be non-covered benefits, providers may not charge the member for those services unless the member has been informed and provided prior written agreement to assume financial responsibility.

Subcutaneous or implantable tibial nerve stimulation systems (for example, the eCoin implantable peripheral neurostimulator) are considered not covered/not medically necessary for all indications, including non‑neurogenic urinary dysfunction such as overactive bladder, due to insufficient evidence of improvement in net health outcome. Similarly, the evidence for transcutaneous tibial nerve stimulation is insufficient to demonstrate a clear clinical benefit for urge urinary incontinence and urinary urgency in the available trials.
