Coverage and Medical Necessity Criteria

Adult HNS medical necessity

Hypoglossal nerve stimulation (HNS) with the Inspire FDA‑approved device may be considered medically necessary when ALL of the following criteria are met:

Adult HNS eligibility: Age ≥ 18 years

AHI: Apnea‑Hypopnea Index (AHI) ≥ 15 and ≤ 100AHI >=15 and <=100

Central apneas: Less than 25% central apneas<25% central apneas

CPAP failure or intolerance: CPAP failure (residual AHI ≥ 15 OR failure to use CPAP ≥ 4 hours/night for ≥ 5 nights/week) OR inability to tolerate CPAP

BMI limit: Body mass index ≤ 35 kg/m2BMI <=35 kg/m2

Some study populations reference BMI ≤ 32 kg/m2 as selection criteria

Palatal collapse: Absence of complete concentric collapse at the soft palate level (favorable palatal collapse pattern)

Adolescent (Down syndrome) HNS medical necessity

HNS may be considered medically necessary in adolescents/young adults with Down syndrome when ALL of the following are met:

Age 13 to 18 years (document also references adolescents/young adults age 10–21 in some sections) with Down syndrome

Document references varying age ranges in different sections

AHI: Apnea‑Hypopnea Index (AHI) > 10 and < 50 (with <25% central apneas) after prior adenotonsillectomyAHI >10 and <50

Alternative: Individual has a tracheotomy OR is ineffectively treated with CPAP due to noncompliance, discomfort, undesirable side effects, persistent symptoms despite compliance, or refusal to use the device

BMI: Body mass index ≤ 95th percentile for age

Minimally invasive procedures for treatment of obstructive sleep apnea (OSA) or snoring — including radiofrequency volumetric tissue reduction of the tongue or palate, laser-assisted uvulopalatoplasty (LAUP), and palatal stiffening procedures (for example, cautery-assisted palatal stiffening, sclerosing-agent injection, or palatal implant insertion) — are not covered / not medically necessary because the evidence is insufficient to determine effects on health outcomes. Tongue base suspension and other minimally invasive tongue-base procedures are specifically listed as not covered/not medically necessary. These interventions are also considered not covered for treatment of isolated snoring in the absence of documented OSA, as snoring alone is not a covered medical condition.

The following procedure codes are explicitly designated as not covered / not medically necessary for Medicare Advantage Plans and are also listed as not medically necessary for Commercial Products: 41512, 41530, S2080, and C9727. Claims submitted using these codes for OSA or snoring will be denied or considered not medically necessary; for procedures without a specific CPT code, file with an appropriate Unlisted Procedure code.

Hypoglossal nerve stimulation (HNS) using FDA‑approved devices other than the Inspire device (for example, Genio) is considered not medically necessary for treatment of clinically significant OSA when the indication and clinical circumstances are otherwise similar to those covered for the Inspire device.
