Coverage Criteria for Oral Appliances and Novel OSA Treatments

Intraoral appliance coverage

Covered when ALL of the following are met

Primary coverage for intraoral appliances: Intraoral appliances for treatment of documented mild to moderate obstructive sleep apnea are covered when rendered by clinicians trained in oral sleep appliance management.mild-to-moderate OSA

Appliances are billed under the member's durable medical equipment benefit; the appliance and related fitting must be provided by a dentist or orthodontist experienced in making these devices.

Included services in global fee: Initial evaluation, oral/dental impressions, fabrication of the appliance, initial fitting/education, and three follow-up visits are included in the global fee for the device.three follow-up visits

Cephalometric X-rays (with and without the appliance) may be billed separately as diagnostic tests. Additional visits beyond the three follow-up visits are the member's responsibility unless an additional device/replacement is required.

Novel OSA treatments — coverage stance

Novel OSA devices not covered or not medically necessary: Nasal expiratory positive airway pressure (EPAP), oral pressure therapy (OPT), palate and mandible expansion devices (e.g., DNA and mRNA appliances), daytime electrical stimulation of the tongue, and sleep positioning trainers with vibration are considered not covered or not medically necessary due to insufficient evidence of net health benefit.insufficient evidence

Specific devices cited include PROVENT, Winx, NightBalance, eXciteOSA, DNA and mRNA appliances; further well‑designed trials are needed to evaluate these therapies.

Oral appliances used to treat conditions other than obstructive sleep apnea (for example, temporomandibular joint disease or bruxism) are not covered for all product lines. Over-the-counter oral appliances for OSA are also not covered because they have not demonstrated equivalent effectiveness to custom-fitted devices. Devices intended solely for treatment of snoring that is not associated with obstructive sleep apnea are not covered. Specific device categories identified as not covered or of insufficient evidence include nasal expiratory positive airway pressure (EPAP) and oral pressure therapy devices, palate and mandible expansion appliances, daytime electrical stimulation of the tongue, and sleep positioning trainers with vibration.

If services are determined to be not medically necessary or otherwise a non-covered benefit, providers may not bill the member unless the member was informed in writing and provided advance agreement to pay out-of-pocket.
