Coverage for prevention of oral mucositis

Covered when ALL of the following are met:

Prevention of oral mucositis coverage: Patient is undergoing cancer treatment associated with increased risk of oral mucositis (including chemotherapy and/or radiotherapy and/or hematopoietic stem cell transplantation).

Clinical evidence: For individuals at increased risk of oral mucositis due to cancer treatment and/or hematopoietic cell transplantation, evidence demonstrates a meaningful improvement in net health outcome with LLLT when used for prevention of oral mucositis.

LLLT is not covered / not medically necessary for all indications other than prevention of oral mucositis. Examples of indications for which LLLT is not covered include: carpal tunnel syndrome; neck pain; subacromial impingement; temporomandibular joint pain; adhesive capsulitis; low back pain; osteoarthritic knee pain; rheumatoid arthritis; heel pain (eg, Achilles tendinopathy, plantar fasciitis); Bell palsy; wound healing; fibromyalgia; and lymphedema.

For BlueCHiP for Medicare and for Commercial products, low-level laser therapy (LLLT) is covered only for the prevention of oral mucositis in patients undergoing cancer treatment associated with increased risk of oral mucositis (including chemotherapy and/or radiotherapy and/or hematopoietic stem cell transplantation). LLLT is not medically necessary for all other indications because the evidence is insufficient to determine its effects on health outcomes.

Low-level laser therapy (LLLT) is covered only for prevention of oral mucositis in patients undergoing cancer treatment associated with increased risk of oral mucositis (eg, chemotherapy, radiotherapy, and/or hematopoietic stem cell transplantation). Low-level laser therapy (LLLT), also called photobiomodulation, uses red-beam or near-infrared lasers with a wavelength between 600 and 1000 nm and low power settings (typically in the range of 5–500 mW) to produce photobiostimulation without thermal injury.
