Coverage Criteria for Microprocessor-Controlled Prostheses

Medical Necessity for Microprocessor-Controlled Knee (Transfemoral Amputation)

Covered when ALL of the following are met:

Primary candidacy requirements: Individual has transfemoral amputation and meets physical and cognitive requirements including adequate cardiovascular and pulmonary reserve for ambulation at faster than normal walking speed; adequate strength and balance to activate the knee unit; and adequate cognitive ability to master use and care of the technology.

Functional need (at least one required): Demonstrated need for long-distance ambulation at variable rates OR demonstrated need for ambulation on uneven terrain OR regular use on stairs (limited home stair/ambulation is insufficient). Typically daily and frequent need for two or more of these activities supports benefit.daily/frequent; often 2+ activities

Daily ambulation of at least 400 continuous yards, daily and frequent ambulation at variable cadence or on uneven terrain, and daily/frequent use of ramps/stairs (especially descent) should be considered.

Fitting and wound status: Stable wound and ability to fit the socket; immediate postoperative fit may be possible for new amputees if other criteria are met.

Not excluded by contra-indications: Not appropriate when inability to use swing/stance features, poor balance or ataxia limiting ambulation, significant hip flexion contracture, limited cardiovascular/pulmonary reserve or profound weakness, significant deformity impairing stride, limited cognitive ability, falls outside manufacturer weight/height limits, extreme environmental conditions preventing maintenance/charging, or for long-distance/competitive running.

For Medicare Advantage Plans and Commercial Products, provision of a powered knee is explicitly addressed in the policy. The policy states that a powered knee is considered not covered for Medicare Advantage Plans and not medically necessary for Commercial Products because the evidence is insufficient to show improvement in net health outcome.
