Medical Necessity Criteria for IRF Admission

Covered when ALL of the following are met:

Admission readiness and benefit: Documentation at admission shows reasonable expectation that the patient can actively participate in, and significantly benefit from, an intensive rehabilitation therapy program

Physician supervision: Patient's medical management and rehabilitation needs require an inpatient stay and close physician involvement, demonstrated by documented face-to-face visits from a rehabilitation physician at least 3 times per week throughout the IRF stay; beginning with week two, one of the three weekly visits may be conducted by a qualified non-physician practitioner as determined by the IRF and within state scope of practice; in week one all three visits must be by the rehabilitation physician>=3 visits/week

Coverage for acute inpatient rehabilitation facility (IRF) admission is provided only when the medical record at the time of admission documents that the patient meets all medical necessity criteria. Specifically, the record must show a reasonable expectation that the patient can actively participate in and significantly benefit from an intensive rehabilitation therapy program; that the patient’s medical management and rehabilitation needs require an inpatient level of care with close physician supervision (face-to-face rehabilitation physician visits at least 3 times per week); that the complexity of nursing, medical, and rehabilitation needs requires an interdisciplinary team with periodic team conferences (minimum once weekly) including required disciplines; and that the record specifies the nature, degree, and expected timeframe for measurable functional improvement with the goal of safe return to home or community within a predetermined, reasonable period of time.

IRF care is not supported when the medical record fails to document the necessary elements for inpatient rehabilitation. Examples of unsupported scenarios include lack of documentation showing a reasonable expectation that the patient can actively participate and benefit from an intensive program, absence of evidence that the patient’s condition requires an inpatient stay with close physician involvement (including the required face-to-face visits), failure to document an interdisciplinary team approach with minimum weekly team conferences and required disciplines represented, or failure to describe the measurable functional improvement expected and the timeframe to achieve it. Admissions lacking these documented elements are at risk for non-coverage or denial.

Medical record entries should describe the patient’s current functional status, cognitive and behavioral ability to participate, anticipated therapy tolerance, and how participation will lead to measurable functional gains within a reasonable timeframe.

The medical record must document physician face-to-face visits by the rehabilitation physician at least three times per week throughout the IRF stay. Beginning in the second week of admission, one of the three required weekly face-to-face visits may be conducted by a non-physician practitioner (e.g., NP or PA) who the IRF determines has specialized training and experience in inpatient rehabilitation and when such duties are within that practitioner’s scope of practice under applicable state law. The purpose of these visits is to assess medical and functional status, document progress, and modify the treatment plan as needed.

Background

The purpose of IRF treatment is to achieve a practical, documentable improvement in the patient’s functional condition sufficient for a safe return to the home or community within a predetermined, reasonable period of time.
