Background & Evidence Summary

Peripheral subcutaneous field stimulation (PSFS), also called peripheral nerve field stimulation or target field stimulation, places leads in subcutaneous tissue within the area of maximal pain to provide diffuse stimulation over painful areas; it is a modification of peripheral nerve stimulation.

The mechanism of action is uncertain; proposed mechanisms include increased endogenous endorphins, modulation of A-delta/C fibers with activation of large-diameter A-beta fibers, local stimulation of cutaneous nerve endings, local anti-inflammatory or membrane-depolarizing effects, or a central action via antegrade activation of A-beta fibers. Permanent implantation is typically preceded by a percutaneous stimulation trial with at least 50% pain reduction.

Common complications reported with PSFS include lead migration, lead breakage, and infection of the lead or neurostimulator. Evidence evaluating PSFS includes small randomized trials, a nonrandomized comparative study, and case series, but overall the evidence is insufficient to establish improvement in net health outcomes.
