Background and Evidence Summary

Percutaneous electrical nerve stimulation (PENS) is performed with needle electrodes inserted around or adjacent to peripheral sensory nerves in the painful area to deliver stimulation. Percutaneous neuromodulation therapy (PNT) is a variant that uses very fine filament needle-like electrode arrays placed near the painful area to stimulate peripheral sensory nerves.

Both PENS and PNT have been evaluated for chronic musculoskeletal or neuropathic pain conditions (examples include low back pain, neck pain, diabetic neuropathy, chronic headache, hyperalgesia, and knee osteoarthritis) and are intended for patients who have failed other treatments.

The highest-quality PENS trial reported no difference in outcomes between the active treatment (30 minutes of stimulation with 10 needles) and a sham procedure (5 minutes of stimulation with 2 needles).

PNT evidence includes a single randomized controlled trial that is limited by lack of investigator blinding, unclear participant blinding, and short-term follow-up. Taken together, the trial record is small and methodologically limited, and the evidence is judged insufficient to support commercial coverage.
