Background & Evidence

The vestibular system includes five end organs — three semicircular canals that sense head rotations and two otolith organs (saccule and utricle) that sense gravity and linear acceleration — and is a key component in balance control. Vertigo and imbalance may arise from peripheral (labyrinth, vestibular nerve) or central (brainstem) causes.

Benign paroxysmal positional vertigo (BPPV) is commonly caused by loose otoconia entering a semicircular canal (most often the posterior canal). BPPV is typically diagnosed clinically from a compatible history, positional testing such as the Dix-Hallpike or lateral roll tests, and resolution of symptoms with canalith repositioning maneuvers; laboratory testing generally does not add diagnostic value in typical BPPV.

When vertigo cannot be attributed to BPPV by history or standard maneuvers, laboratory-based tests based on the vestibulo-ocular reflex may help distinguish central versus peripheral causes and unilateral versus bilateral deficits. Established tests include ENG/VNG test batteries (oculomotor and positional testing; ENG uses electrodes and VNG uses infrared video goggles), caloric testing (warm or cold irrigation of each external ear canal to evaluate unilateral function), and rotational chair testing (computer-driven chair to assess bilateral vestibular function).

VEMP testing (cervical and ocular) uses loud sounds or bone vibration to assess otolith function; however, evidence on diagnostic accuracy and clinical utility is currently insufficient to determine health outcome benefits, so VEMP is not considered covered.
