Medically necessary vestibular testing

Covered when ALL of the following are met

Clinical indication: Patient has symptoms of a vestibular disorder (eg, dizziness, vertigo, imbalance)

Supported by policy statements for commercial and general coverage

Prior clinical evaluation: A clinical evaluation, including maneuvers such as the Dix–Hallpike test when indicated, has been performed and has failed to identify the cause of the symptoms

Laboratory testing is intended for cases not attributable to BPPV by history/maneuvers

Test selection: Testing is limited to established laboratory-based tests (ENG/VNG test batteries, caloric testing, rotational chair testing)

These tests assess the vestibulo-ocular reflex to distinguish central vs peripheral etiologies and unilateral vs bilateral deficits

Laboratory-based vestibular function tests (such as ENG/VNG batteries, caloric testing, and rotational chair testing) are not covered when used to assess typical benign paroxysmal positional vertigo (BPPV) that can be diagnosed and managed with standard clinical positional maneuvers (for example, Dix–Hallpike or lateral roll) and canalith repositioning. The policy specifies that when the history, symptoms, and response to standard maneuvers attribute vertigo to BPPV, laboratory testing does not add diagnostic value and is therefore not covered.

Additionally, repeat vestibular function testing after successful treatment resolution of symptoms is not covered. The policy also states that laboratory-based vestibular tests that are not described in this document are considered not covered / not medically necessary because current evidence is insufficient to demonstrate improved net health outcomes.

Vestibular evoked myogenic potential (VEMP) testing (including cervical cVEMP and ocular oVEMP) is considered not covered / not medically necessary. The policy states that VEMP techniques are newer tests using loud sound or bone vibration to assess otolith (saccule/utricle) function, but the evidence is insufficient to show that VEMP testing improves net health outcomes.

Because there is no specific CPT code for VEMP, claims for VEMP should be filed using an unlisted otorhinolaryngologic procedure code (eg, 92700), and such testing is treated as not medically necessary under this policy.

Stepwise evaluation before laboratory testing

When benign paroxysmal positional vertigo (BPPV) is suspected, perform clinical evaluation maneuvers (eg, Dix-Hallpike, lateral roll test) and canalith repositioning before ordering laboratory-based vestibular testing. Laboratory testing (ENG/VNG, caloric testing, rotational chair) is appropriate when clinical evaluation fails to identify the cause of symptoms or when history/response to maneuvers does not support typical BPPV.

Background

The vestibular system comprises three semicircular canals and two otolith organs (saccule and utricle). It provides sensory input about head motion and orientation and works with visual and somatosensory systems to maintain balance and gaze stability. Dysfunction of the vestibular system commonly presents as dizziness, vertigo, imbalance, nausea, and related functional impairment.

Benign paroxysmal positional vertigo (BPPV) is commonly caused by displaced otoconia affecting a semicircular canal (often the posterior canal). BPPV typically produces characteristic, brief positional vertigo that can be reproduced with positional maneuvers (eg, Dix–Hallpike, lateral roll) and is frequently treatable with canalith repositioning maneuvers. Laboratory vestibular testing evaluates the vestibulo-ocular reflex and related pathways to help distinguish peripheral from central causes and to identify unilateral versus bilateral deficits, and is intended for cases where clinical evaluation and maneuvers do not explain the symptoms.
