Medically Necessary Indications

Measurements of CA 125 may be considered medically necessary in patients when ANY of the following apply:

Symptoms suggestive of ovarian cancer

Supported by policy statement that CA 125 may be considered medically necessary in patients with symptoms suggestive of ovarian cancer (chunk 4).

Known malignant disease with prior elevation: Known ovarian cancer or other gynecologic malignancy in which baseline CA 125 has been shown to be elevated

Supported by statement that CA 125 may be used in patients with known ovarian cancer or other gynecologic malignancies with previously elevated baseline (chunks 4,5).

Pre-operative baseline for suspicious pelvic mass: Suspicious pelvic mass preoperatively to establish a baseline for postoperative monitoring

Supported by indication for obtaining CA 125 as part of initial pre-operative workup to use as baseline for postoperative monitoring (chunk 5,7).

Monitoring treatment response or surveillance: Management and treatment follow-up after initial surgery and/or chemotherapy, monitoring response of advanced or recurrent disease after therapy, and routine surveillance with periodic testing schedule

Supported by indications for use in management after surgery/chemotherapy, monitoring recurrent/advanced disease response, and surveillance schedule (chunks 5,7).

Clinical Indication Documentation

When ordering CA 125 for pre-operative baseline measurement, monitoring after surgery and/or chemotherapy, or surveillance/response assessment in patients with known or suspected gynecologic malignancy, document the clinical indication in the medical record. Documentation should include the presenting symptoms or diagnosis, prior CA 125 levels (if applicable), intended use of the result (baseline, monitoring response, or surveillance), and treatment course or planned interventions.

Use for pre-operative baseline when a suspicious pelvic mass is present and CA 125 is known to be elevated in the patient population.
Use for monitoring after initial surgery and/or chemotherapy to assess treatment response (including obtaining serial measurements to calculate CA 125 half-life when clinically indicated).
Use for surveillance/response assessment in advanced or recurrent disease, with documentation of treatment cycles and planned frequency of testing.

Denial Risk

Screening in Asymptomatic Patients

CA 125 measurement is not covered for BlueCHiP for Medicare and is not medically necessary for Commercial products when used as a screening test in asymptomatic patients for ovarian cancer.

Screening asymptomatic patients for ovarian cancer with CA 125 is not supported by evidence showing mortality benefit and leads to unnecessary invasive procedures from false positives.

Denial Risk

Differential Diagnosis for Pelvic Mass

CA 125 is specifically not covered for aiding in the differential diagnosis of patients with a pelvic mass because the test's sensitivity and specificity are insufficient to reliably distinguish benign from malignant masses.

CA 125 should not be used alone to determine the etiology of a pelvic mass; evaluation should rely on clinical assessment and imaging, with pathology as indicated.
