Navigated transcranial magnetic stimulation (nTMS) is a noninvasive cortical mapping technique in which magnetic pulses are delivered to the scalp while a neuronavigation system registers stimulation sites to the patient’s MRI; responses such as surface electromyography (for motor mapping) or disruption of speech tasks (for language mapping) are used to infer cortical motor and language areas (nTMS is defined as a neuronavigated, MRI‑guided TMS procedure).

Direct cortical stimulation (DCS) performed intraoperatively during craniotomy remains the reference standard for mapping eloquent cortex; DCS directly stimulates the exposed cortical surface to identify essential functional areas and is considered the most accurate method for preserving function during tumor resection.

The published literature consists largely of observational studies and case series comparing nTMS mapping to DCS or to historical controls. Results have been variable: studies report distances between nTMS and DCS hotspots, but limited verification (only exposed cortex can be confirmed with DCS) and methodological limitations produce mixed accuracy metrics. Controlled observational studies of clinical outcomes have had inconsistent findings (some report fewer postoperative deficits or improved planning with nTMS, while others show no difference in survival or outcomes).

Because of the limited, nonrandomized evidence and variable accuracy—including reports of high false‑positive rates for language mapping and inconsistent impact on clinical outcomes—the evidence is insufficient to conclude that nTMS improves health outcomes. Accordingly, nTMS is considered investigational/not medically necessary for preoperative localization of eloquent brain areas and similar indications.
