PathfinderTG testing is a selective, second-line diagnostic supplement and is considered medically reasonable and necessary only when a comprehensive first-line evaluation has been completed and clinical uncertainty remains about malignancy or malignant potential of a pancreatic cyst. Testing is appropriate only when a treatment decision (for example, surgery) has not already been made and the molecular results will influence that decision.

Maintain a legible medical record that includes patient identification, dates of service, and the legible signature of the ordering clinician. Background

Pancreatic cysts are fluid-filled, cell-lined lesions — most commonly mucinous cysts such as intraductal papillary mucinous neoplasms (IPMN) and mucinous cystic neoplasms — that carry a variable risk of progression to pancreatic cancer and are followed to assess that risk. Barrett esophagus is a metaplastic change of the esophagus that can progress to adenocarcinoma, and solid pancreaticobiliary lesions have a broad differential that includes primary and metastatic cancers, lymphoma, and inflammatory conditions.

Topographic genotyping (PathFinderTG, also referenced by proprietary panels such as PancraGEN and BarreGEN) integrates microscopic anatomic pathology with molecular tissue analysis, using microdissection to increase tumor cell yield for mutational analysis. In this policy the platform is described as a specialized, laboratory-developed molecular test intended as a second-line diagnostic adjunct when first-line histology/cytology and radiographic evaluation are inconclusive and additional molecular information may influence management.
