Coverage Criteria

Medically necessary coverage conditions

Medically necessary coverage conditions: 1) A concise, documented statement in the medical record that a decision regarding treatment (e.g., surgery) has NOT already been made and that PathfinderTG results will assist in determining whether more aggressive treatment is necessary; 2) Comprehensive first-line diagnostic evaluation was performed and remains inconclusive regarding current malignancy or malignant potential of the pancreatic cyst; 3) The test is being requested as a selective, occasional second-line diagnostic supplement (PathfinderTG used adjunctively after first-line tests); 4) Prior authorization obtained when required by the member's plan.

Prior Authorization Required

Prior authorization is required for Medicare Advantage plans and recommended for Commercial products. Authorization must be obtained by the ordering physician via the payer's online tool for participating providers. Laboratories, laboratory representatives, or third parties may not obtain authorization, initiate appeals, or otherwise act on behalf of the ordering physician. Unauthorized laboratory services will be denied and are the financial responsibility of the provider performing the service.

The decision to use this test must be made before a definitive treatment decision (e.g., surgery) has been taken.

Eligibility Requirements

Mucinous cystic neoplasm (MCN)

Background

Pancreatic cysts are fluid-filled, often mucinous lesions such as intraductal papillary mucinous neoplasms (IPMN) and mucinous cystic neoplasms that carry variable risk for malignant transformation. Mucinous cysts are associated with future pancreatic cancer risk, and evaluation aims to distinguish benign from premalignant or malignant lesions to guide surveillance or surgery.
