Diagnostic evaluation must document a comprehensive approach to exclude other causes of groin pain. This should include a detailed history, focused physical examination, and appropriate imaging (for example MRI or ultrasound) to evaluate soft-tissue injury and to exclude alternative diagnoses such as stress fracture, femoroacetabular impingement, or labral tear. Documentation should clearly describe symptoms, activity-related exacerbating factors, physical exam findings that rule out a standard inguinal hernia or muscle strain, and imaging results.

Conservative Therapy Before Surgery

Conservative, nonoperative management should be attempted and documented prior to considering surgical treatment. Conservative therapy may include activity modification, physical therapy focused on core and hip strengthening, nonsteroidal anti-inflammatory medication as appropriate, and targeted rehabilitation for hip range-of-motion limitations when present. Documentation should state the duration and components of conservative care and the degree of symptomatic improvement (or lack thereof).

Athletic pubalgia, often termed sports hernia or core muscle injury, is an activity-related syndrome of lower abdominal and groin pain that typically affects high-performance athletes in sports involving twisting and cutting. Proposed causes include occult defects of the transversalis fascia, soft-tissue injury at the rectus abdominis or adductor insertions, osteitis pubis, and nerve entrapment; femoroacetabular impingement (FAI) may coexist and contribute. Diagnosis is based on clinical history and physical examination and is commonly supported by imaging such as MRI or ultrasound to exclude other sources of groin pain.
