Medical necessity criteria

Commercial product requirement: Product is a Commercial product (policy applies to Commercial products only)

Covered indications for malabsorption: Physician-ordered oral nutritional formulas or special medical formulas for home use are covered when medically necessary for treatment of malabsorption caused by: Crohn's Disease OR Ulcerative Colitis OR Gastroesophageal Reflux OR Chronic Intestinal Pseudo-Obstruction OR Inherited Diseases of Amino Acids and Organic Acids

Low protein foods for inborn errors: Low protein food products are covered when ordered by a physician for individuals with inherited diseases of amino acids and organic acids (e.g., Phenylketonuria [PKU], Tyrosinemia, Homocystinuria, Maple Syrup Urine Disease, Propionic Aciduria, Methylmalonic Aciduria)

Infant milk/soy allergy rule: For infants aged 0–12 months with milk/soy protein allergy, oral nutritional or special medical formulas are covered when there is failure to thrive AND blood in the stoolsage 0-12 months + both clinical findings required

This policy does not cover donor breast milk therapy; donor breast milk is expressly stated as not a covered service under Blue Cross & Blue Shield of Rhode Island plans. Additionally, the policy does not address enteral nutrition therapy when used as the sole source of nutrition delivered through a feeding tube — those services are managed under the related Enteral/Parenteral Nutrition Therapy policy. Requests that fail to meet the criteria may be denied, and only services that meet the documented physician order and medical necessity requirements will be covered.

After preauthorization approval, members seeking reimbursement must submit itemized receipts that clearly identify (highlight or circle) the special food products purchased, together with the Oral Enteral Food Products Reimbursement Form (or the reimbursement form listing the special foods purchased). Receipts and the completed form must be submitted within one year from the date of purchase to the Claims Department at the address below.
