Catheter-based impedance pH monitoring using CPT codes 91037 and 91038 is explicitly not covered for Medicare Advantage Plans and is considered not medically necessary for Commercial Products. These codes describe nasal catheter intraluminal impedance electrode placement with recording, analysis, and interpretation (91037) and the prolonged intraluminal impedance monitoring service (91038). Claims submitted with these CPT codes will be denied for Medicare Advantage members and are considered not medically necessary for Commercial members under this policy.

The evidence base for esophageal pH monitoring (catheter-based or wireless) and impedance-pH monitoring is limited. Cross-sectional and diagnostic performance studies show correlation between positive tests and endoscopically defined GERD or GERD symptoms, and some studies report higher positive rates with prolonged wireless monitoring or with impedance testing versus pH testing alone. However, there is no clear reference standard for clinical GERD, diagnostic characteristics cannot be definitively established, and importantly there are no studies demonstrating clinical utility (improved patient health outcomes) from these tests. Because the chain of evidence linking test results to improved outcomes is weak, the evidence is considered insufficient to determine that these technologies improve net health outcomes—supporting the cautious coverage stance and the noncoverage decision for catheter-based impedance testing in certain products.

Prior Medical Management

Trial of medical management (for example, a therapeutic trial of proton pump inhibitor [PPI]) is the usual initial approach to suspected GERD. Endoscopy is indicated for patients who do not respond to medical therapy or who have recurrent/chronic symptoms to confirm diagnosis and assess severity. If endoscopy is nondiagnostic or discordant with clinical evaluation, further diagnostic testing (such as pH monitoring) may be considered.

Background

Esophageal pH monitoring measures acid exposure in the lower esophagus to help evaluate gastroesophageal reflux disease (GERD) when diagnosis remains uncertain after clinical assessment and endoscopy. Catheter-based systems typically record pH continuously for approximately 24 hours, with a nasal catheter placed for electrode recording, analysis, and interpretation. Wireless pH monitoring uses a mucosal-attached telemetry pH electrode (pH capsule) that can record and transmit pH data for an extended period — commonly up to 96 hours — which may increase diagnostic yield in some patients.
