Medical necessity for malabsorption: Used for treatment of malabsorption caused by Crohn's disease OR Ulcerative Colitis OR Gastroesophageal Reflux OR Chronic Intestinal Pseudo-Obstruction OR Inherited Diseases of Amino Acids and Organic Acids

Any one of the listed causes of malabsorption qualifies

Inherited metabolic disorders or infant allergy: Low protein foods covered for inherited diseases of amino acids and organic acids; OR formulas covered for milk/soy protein allergy in infants 0-12 months with failure to thrive AND blood in the stools

This policy excludes coverage for donor breast milk therapy; donor breast milk is explicitly not a covered service under Blue Cross & Blue Shield of Rhode Island plans. Additionally, this policy does not address enteral tube‑feeding as the sole source of nutrition delivered through a feeding tube — such care is managed under the related Enteral/Parenteral Nutrition Therapy policy. When the specific clinical criteria in this policy are not met, low protein food products, oral nutritional formulas, and special medical formulas are considered non‑covered and a contractual exclusion for all Commercial products. Coverage for these products is limited to the conditions and physician‑ordered indications stated in the policy; services that do not meet those criteria are excluded from coverage.

Providers must ensure members are informed of the reimbursement process following preauthorization approval. When members seek reimbursement, advise them to submit itemized receipt(s) that clearly identify the special foods purchased and to complete the required reimbursement form. Receipts and the reimbursement form must be submitted within one year from the date of purchase to the address below.
