Fecal calprotectin is a calcium- and zinc-binding protein released from neutrophils and used as a stool biomarker of intestinal neutrophilic inflammation. It may help distinguish organic inflammatory conditions (eg, IBD) from noninflammatory conditions (eg, IBS).

Compared with serologic markers, fecal markers like calprotectin are more specific to GI inflammation because they are not elevated in extra-digestive inflammatory processes. Calprotectin comprises about 60% of neutrophil cytoplasmic proteins and is released during neutrophil activation or cell death.

A practical advantage of fecal calprotectin is specimen stability: it has been shown to be stable in feces at room temperature for up to 1 week, which facilitates home collection and mailing to a laboratory. Limitations include increases after NSAID use, variation with age, possible elevation with bleeding, and uncertainty about optimal diagnostic cutoffs.

Clinical uses studied include: differentiating IBD from noninflammatory bowel disease and selecting patients for endoscopy (most studies used a 50 µg/g cutoff as the recommended lower bound), monitoring disease activity, and predicting relapse (meta-analyses reported an optimal monitoring cutoff near 152 µg/g with pooled sensitivity ~72% and specificity ~74%), though evidence is sufficient for diagnostic use but insufficient to support improved health outcomes for management or surveillance.
