Commercial not medically necessary: CPT codes 77078, 76977, G0130, 0508T, 0554T, 0555T, 0557T, 0556T, and 0558T are listed as not medically necessary for Commercial Products. (Note: 0508T is an exception for Medicare Advantage as noted separately.)

These modalities (CT-based QCT, ultrasound densitometry, SEXA, and finite element analysis using CT data) are considered not medically necessary for Commercial Products per this policy.

Several bone density and related modalities are discussed in this policy. Computed tomography bone mineral density (QCT) refers to CT-derived measures of bone mineral density and related finite element analyses; the policy states these studies are considered not medically necessary for Commercial Products because evidence is insufficient to determine impact on health outcomes. Ultrasound bone density measurement (including pulse-echo ultrasound) and single energy X-ray absorptiometry (SEXA) are likewise described as having insufficient evidence and are considered not medically necessary for Commercial Products. Older photon absorptiometry techniques (single-photon [SPA] and dual-photon [DPA] absorptiometry) are also listed as not medically necessary.

Clinical Background

Osteoporosis is common in older adults due to age- and menopause-related bone loss and measuring bone mineral density helps identify individuals at increased fracture risk and to monitor treatment. Central DXA of the hip and spine is the preferred test because it is the most predictive of fragility fractures and is used for diagnosis and repeat assessment. Alternative peripheral or investigational technologies have not demonstrated consistent evidence of improving important health outcomes compared with central DXA.
