Coverage Criteria for Bone Density Studies

Indications for initial central DXA

Covered when ALL of the following are met

Initial central DXA for fracture risk

Age-based screening thresholds: Women age >=65 years; Men age >=70 years;
Age and risk-based testing: Younger postmenopausal women with concern based on risk factors; Men age 50-70 with concern based on risk factors;
Clinical conditions or medications: Adults with a condition or medication associated with low bone mass or bone loss (eg, rheumatoid arthritis, long-term glucocorticoid use, hypogonadism, untreated long-standing hyperthyroidism, chronic malabsorption).

Peripheral BMD testing indications

Peripheral measurement may be appropriate when ANY of the following apply

Appropriate uses of peripheral BMD testing

Condition-specific indication: Hyperparathyroidism, where forearm measurement is essential for diagnosis.
Technical or patient limitations: When central hip and/or spine DXA cannot be performed (eg, body habitus/table weight limits) or the hip/spine cannot be imaged.

The policy distinguishes measurement modalities by clinical utility and evidence. Central DXA (dual-energy X-ray absorptiometry) of the axial skeleton (eg, spine and hip), including vertebral fracture assessment when appropriate, is the preferred modality for diagnosis and monitoring of osteoporosis and fracture risk because it evaluates central sites most predictive of fragility fracture. By contrast, ultrasound densitometry (including pulse-echo ultrasound), single-photon absorptiometry (SPA), dual-photon absorptiometry (DPA), and certain single-energy absorptiometry techniques are considered not medically necessary or not covered because current evidence is insufficient to demonstrate meaningful effects on health outcomes compared with central DXA. These modalities should not be used as a substitute for central DXA when diagnosis or management decisions depend on central bone mineral density measurements.

Coverage for bone density studies is subject to member-specific benefits and the subscriber agreement. Providers must verify eligibility and benefit details for each member prior to performing or billing for services; this policy is informational and does not guarantee payment.

When a service is medically necessary but identified as a non-covered benefit under a member's plan, providers may not bill the member for that service unless the member has been informed and has provided a written agreement to accept financial responsibility in advance.

Alternative modalities have limited roles. Peripheral BMD testing (eg, forearm, wrist, heel) may be appropriate in specific circumstances such as evaluation of primary hyperparathyroidism (forearm), when central hip/spine DXA cannot be performed, or when the patient exceeds table weight limits. However, peripheral results are not a substitute for central DXA for diagnostic or monitoring purposes.

The following modalities are explicitly not supported for routine diagnosis or management due to insufficient evidence: ultrasound densitometry (including pulse-echo), SPA, DPA, and certain single-energy absorptiometry techniques. Claims for these modalities may be denied or determined not medically necessary.

Background and Clinical Context

Osteoporosis is common in older adults and increases risk of fragility fractures. Measurement of bone mineral density by central DXA at the hip and spine is the most predictive test for fracture risk and is the standard for diagnosing osteoporosis and guiding pharmacologic therapy. Clinical risk assessment tools such as FRAX may be used with or without BMD to estimate fracture risk and inform testing decisions and treatment.

Treatment (lifestyle interventions and pharmacologic therapy) has been shown to reduce fracture risk in patients with osteoporosis-range BMD or prior fragility fractures; therefore, accurate assessment of central BMD is important to identify patients who will benefit from therapy.
