Clinical Rationale and Context

Bone turnover markers are biochemical measures of bone formation or bone resorption that can be detected in serum or urine using assays such as immunoassay or high-performance liquid chromatography. Examples include tests for collagen cross-links and osteocalcin and these markers are proposed as adjuncts to bone mineral density (BMD) testing for diagnosis, fracture risk assessment, and for earlier assessment of response to therapy (treatment-related changes in BMD occur slowly).

Although marker levels may change within months and therefore can provide a more immediate indication of biological response to therapy, the evidence is insufficient to show that measuring bone turnover markers improves patient management or health outcomes. Studies to date do not demonstrate consistent improvements in treatment decisions, adherence, or reduced fracture rates based on routine measurement of these markers.
