Medically necessary conditions

Air or water ambulance is considered medically necessary when:

Primary necessity conditions: Either the time needed to transport the patient by land or the instability of land transport poses a threat to the patient's condition or survival, OR the patient must be transported for treatment to the nearest appropriate hospital capable of providing the required care and the type of physician or specialist needed to treat the patient's condition.

At least one of these must be true

Additional required conditions: AND the proper equipment needed to treat the patient is not available from a ground ambulance, AND the patient needs transfer from one hospital to the nearest appropriate hospital because the transferring hospital lacks the appropriate facilities (for example, trauma unit, burn unit, or cardiac care unit).

Both conditions required in addition to one primary necessity condition

Summary of non-covered or limited transport reasons: Transport to a facility that is not an acute care hospital is not covered. Other non-covered scenarios include transport on commercial or charter flights that are not certified air ambulances, return transportation home by air, water, or ground, and repatriation or medical evacuation services for transport back to the United States. These limitations apply regardless of clinical urgency when the listed conditions are present.

Transportation provided solely for continuity of care is not covered. Examples include when a member requests transfer to their own physician or a specific specialist who is not located at the nearest appropriate hospital. Coverage is limited to transport to the nearest appropriate facility capable of providing the required care.

Documentation should clearly support medical necessity for the transport, including clinical condition, reason transport was required, origin and destination, and why a lower level of transport was not appropriate. For urgent or emergent transports where prior authorization was not feasible, providers must submit complete medical records and transport documentation for retrospective review. Failure to provide adequate documentation may result in denial of payment.
