Medical necessity overview

Primary justifications (ANY of the following)
- The time needed to transport a patient by land or the instability of transportation by land poses a threat to the patient's condition or survival.
- The patient must be transported to the nearest appropriate hospital that is capable of providing the required level of care for the patient's illness and that has the appropriate physician available.
- The proper equipment needed to treat the patient is not available on a ground ambulance.
- The patient must be transferred from one hospital to the nearest appropriate hospital because the transferring hospital does not have the appropriate facilities to provide the medical services needed (for example, trauma unit, burn unit, or cardiac care unit).

Operational note: Meeting any one of the 'Primary justifications' supports medical necessity; documentation must demonstrate which justification applies and why ground transport is insufficient.

Services that are excluded from coverage under this policy include transportation provided solely for continuity of care (for example, to see a preferred physician who is not at the nearest appropriate hospital), transport to a facility that is not an acute care hospital, and return air, water, or ground transportation home.

Documentation and Retrospective Review

Documentation must support the urgency and medical necessity of air or water ambulance transport. For all BCBSRI products, HCPCS air ambulance claims and supporting medical records will be subject to retrospective review; the documentation should clearly demonstrate that ground transport was not appropriate due to time sensitivity, instability for ground transport, lack of required equipment or capability, or that transfer to the nearest appropriate facility was required.

Air ambulance refers to transportation by helicopter or fixed‑wing aircraft that meet certified ambulance and crew standards; water ambulance refers to boats specially designed and equipped to transport the sick or injured and meeting applicable safety regulations. These services are intended when ground transport time or the instability of ground transport endangers the patient, when the nearest appropriate facility with required services or physician is only accessible by air or water, or when necessary treatment equipment is not available on a ground ambulance. Aircraft and crews are expected to meet applicable certification and safety requirements for ambulance operations.
