Such deep or structural vascular abnormalities are considered medically necessary when surgery is indicated and are covered under the member’s surgical benefits; surgical management and coverage decisions for those lesions follow standard medical necessity and surgical policy, not this laser-treatment policy.

Laser treatment for port‑wine stains, hemangiomas, or superficial vascular malformations performed solely to alter or enhance appearance, and where the lesion does not interfere with physical body function, is not covered as it is considered cosmetic.

Background

Vascular birthmarks include hemangiomas and vascular malformations. Hemangiomas are benign endothelial tumors that typically proliferate after birth and often involute over years; a minority (approximately 20%) develop complications that may impair function. Vascular malformations are present at birth, grow proportionally with the child, and may be superficial or deep. Port‑wine stains are capillary malformations that commonly begin as pink macules and, if untreated, tend to darken and thicken over time and are frequently located on the face and neck.

Pulse‑dye and similar vascular lasers penetrate only superficially and are intended for superficial vascular proliferative lesions such as port‑wine stains, superficial hemangiomas, and superficial vascular malformations; deep, structural lesions that require surgery are outside the scope of this policy and are managed under surgical indications.
