Periodontal Services: Criteria include osseous surgery when pocket depths are 5 mm or more with radiographic evidence of interproximal bone loss (number of qualifying teeth determines appropriate code D4260/D4261). Scaling and root planing when pocket depths are 4 mm or more or radiographic evidence of calculus and interproximal bone loss (number of qualifying teeth determines appropriate code D4341/D4342). Tissue grafting when attached gingiva is 2 mm or less per treatment site. Periodontal maintenance is limited to 4 per 12 months. Periodontal scaling and root planing limited to once per site per 24 months. Full mouth debridement limited to one per lifetime.
