Orthodontic Coverage Criteria

Orthodontic services are covered only when dentition and at least one of the following anatomic/occlusal criteria are met:

Implants and Fixed Prosthodontics

Orthodontic Services

Orthodontic services not covered or limited:

Not covered orthodontic services: Replacement of lost or missing appliances; repair of damaged orthodontic appliances; services to alter vertical dimension and/or restore or maintain occlusion (including equilibration, periodontal splinting, full mouth rehabilitation and restoration for misalignment of teeth).

These services are not covered under the orthodontic benefit.

Orthodontic proration: If orthodontic treatment began with another carrier or with no prior coverage and BCBSRI covers treatment, the orthodontic benefit maximum will be prorated according to the remaining treatment duration. For members who began treatment under a BCBSRI dental plan and transitioned without disruption, payments follow the terms of the original plan. Payment will not exceed the Blue Cross Dental allowance.

Proration applies to ongoing treatment started under other coverage as described in policy text.

No benefits will be allowed for implant or implant-related services when the arch can be restored with a standard prosthesis or restoration. Providers should submit the required pre-operative imaging such as a panorex or full intraoral series to support restorative options and consultant review.

Only the pediatric oral care services listed in this policy are covered under the member's medical benefit when the benefit plan includes Essential Health Benefits for members from age 0 up to their 19th birthday. Services not listed in this policy are not covered under the member's medical coverage and would be the member's financial responsibility up to the dentist’s charge.

Replacement of lost or missing orthodontic appliances and repair of damaged orthodontic appliances are not covered. These exclusions also apply to services performed primarily to alter vertical dimension or to restore or maintain occlusion (for example: equilibration, periodontal splinting, full mouth rehabilitation, or restoration for misalignment of teeth).

Orthodontic coverage requires that the dentition stage meet the policy prerequisite: services will not be covered when the dentition contains any more primary teeth than the primary second molars. This dentition requirement must be met before consideration of the additional anatomic/occlusal qualifying criteria listed in the policy.

When a service is medically necessary but is identified as a non-covered benefit under this policy, the procedure remains a non-covered item under the member's medical coverage. Providers and members may need to pursue alternate coverage or referral options; any member self-pay for a non-covered service must be arranged with the member and documented according to plan/contract terms.
