Coverage Summary

This policy defines the pediatric oral care services that are covered under the member's medical benefits for children from birth up to the member's 19th birthday when the plan includes Essential Health Benefits (EHBs). The EHB designation became effective January 1, 2014 and pediatric oral and vision care were included in that requirement.

The policy specifies review criteria, required documentation, service limitations, prior authorization requirements, and frequency limits for covered pediatric dental services. Benefits may vary between groups or contracts; members and providers should consult the applicable Benefit Booklet or Subscriber Agreement for group-specific coverage details.

Claims for dental services listed in this policy that are covered as EHB pediatric dental benefits must follow the stated coding and documentation requirements and may require dental consultant review as specified.

Medical Necessity & Review Criteria

General Dental Review Criteria

General dental review criteria used by the dental consultant. Covered services require meeting the following clinical criteria and submission of required documentation for review.

All of the following must be satisfied for consultant review:

Criteria: Sufficient breakdown as demonstrated on a radiograph; periodontally and endodontically sound permanent tooth.

Required documentation: Intra-oral photo (if available); Pre-operative periapical X-ray; Detailed narrative (if applicable).

Post-Service Endodontic Review Criteria

Post-service endodontic review requirements. When post-service review is performed the following clinical findings and documentation are required.

All of the following must be satisfied for post-service endodontic review:

Criteria: Sound periodontal prognosis; Complete fill to the apex of each canal or documented calcification that prevents complete fill.

Required documentation: Pre-operative and post-operative periapical X-rays (a working film may not be substituted for a post-operative film).

Periodontal Procedure Criteria

Periodontal procedure criteria. Coverage depends on qualifying clinical findings (any) and required documentation (all). Pocket depth thresholds determine appropriate procedure codes.

ALL of the following

Qualifying clinical findings
- Scaling and root planing: pocket depths of >= 4 mm or radiographic evidence of calculus and interproximal bone loss (number of teeth with qualifying depths determine appropriate code D4341 or D4342).>= 4 mm
- Tissue grafts: attached gingiva of <= 2 mm per treatment site may qualify for grafting.<= 2 mm
- Osseous surgery (including entry and closure): pocket depths of >= 5 mm and radiographic evidence of interproximal bone loss (number of teeth with qualifying depths determine appropriate code D4260 or D4261).

Implant Services Criteria

Implant services criteria. Coverage limited by restorability of the arch, frequency rules, and required documentation for consultant review and payment.

Operational note: When payment is requested include pre-op and post-op imaging and narrative; absence may lead to denial.

Abutment Tooth Criteria for Implant/Fixed Prosthodontics

Criteria for abutment teeth used for implant or fixed prosthodontic support. All items are required for consultant review.

ALL of the following

Periodontally and endodontically sound permanent abutment teeth.
Pre-operative periapical X-rays of the entire treatment site.
Detailed narrative describing circumstances related to the treatment and justification for abutment selection (recommended if special circumstances).

Note: narrative should describe prognosis, opposing dentition, and rationale for using the abutment tooth.

Oral Surgery Review Criteria

Oral surgery review criteria. Pre-operative imaging and narrative are required for review of oral surgical procedures.

ALL of the following

Pre-operative X-ray of the treatment site.
Detailed narrative (if applicable) describing diagnosis, planned procedure, and any medical considerations.

Operational note: Submit imaging that adequately demonstrates the surgical site; lack of appropriate imaging may delay review.

Orthodontic Coverage Criteria

Orthodontic coverage criteria. Top-level requirements and clinical severity criteria (any) plus documentation required for consultant review. Note dentition requirement: services not covered when dentition contains more primary teeth than primary second molars.

ALL of the following

Dentition requirement: Services will not be covered when the dentition contains any more primary teeth than the primary second molars.
One of the following severe conditions must be met
- Maxillary/mandibular incisor relationship: overjet of >= 9 mm with impingement where the lower incisors are impinging the palate.>= 9 mm
- Anterior crossbite equal to or greater than 5 mm (short-term, interceptive therapy covered only).>= 5 mm

Endodontic Service Limitations

Endodontic service limitations and timing rules. All listed limits apply for coverage determination.

ALL of the following

Partial pulpotomy for apexogenesis (permanent tooth with incomplete root formation): if a root canal is performed within 90 days of the pulpotomy, the pulpotomy will be considered part of the root canal procedure and is not separately covered.<= 90 days
Therapeutic pulpotomy (excluding final restoration): if a root canal is performed within 90 days of the pulpotomy, the pulpotomy will be considered part of the root canal procedure and is not separately covered.<= 90 days
Pulpal therapy (resorbable filling) anterior, primary tooth (excluding final restoration): coverage up to age 6 for primary incisors; limited to once per tooth per lifetime.

Periodontal Service Frequency & Limits

Periodontal service frequency and limits. All frequency limits below apply to coverage decisions.

ALL of the following

Gingivectomy or gingivoplasty: if performed for four or more teeth, coverage per indication; one to three teeth limited to every 36 months.
Gingival procedure including root planing, flap: frequency and scope evaluated per quadrant/site.
Clinical crown lengthening - hard tissue: frequency per clinical need.
Pedicle soft tissue graft: limited to once per graft site per 36 months.
Osseous surgery (including entry and closure), four or more contiguous teeth or bounded tooth spaces per quadrant: limited per clinical indication and frequency rules.

Fixed Prosthodontics Limits

Fixed prosthodontics limits. Coverage is subject to frequency limits and tooth-based restrictions.

ALL of the following

Major restorative services (core buildup, prefabricated post and core, crowns): limited to 1 tooth per 60 months.
One fixed denture (partial) per treatment area per 60 months.

Operational note: Frequency measured from prior provision of same service to the same tooth or treatment area; documentation should include prior treatment dates.

Orthodontic Non-Coverage Criteria

Orthodontic services not covered. Any of the following make orthodontic services non-covered.

ANY of the following

Replacement of lost or missing appliances.
Repair of damaged orthodontic appliances.
Services to alter vertical dimension and/or restore or maintain the occlusion (including but not limited to equilibration, periodontal splinting, full mouth rehabilitation and restoration for misalignment of teeth).

Operational note: These exclusions apply regardless of other orthodontic coverage criteria and are considered not covered services.

Claims for dental services should be filed on CDT forms; if approved, those claims will be processed under the member's medical benefit.

Dental consultant review is required for specific services and providers must submit the required documentation (radiographs, intra-oral and extra-oral photos, narratives, periodontal charts, cephalometric films, etc.) when requested.

Effective January 1, 2014, Qualified Health Plans are required to cover Essential Health Benefits; pediatric services including oral care were defined as EHBs and this policy sets the scope for oral care services for members from age 0 up to the 19th birthday under those plans.

The policy applies to pediatric members when the plan includes EHB coverage, but benefit specifics and availability may vary by group or contract. Providers and members should consult the appropriate Benefit Booklet or Subscriber Agreement for the applicable pediatric dental benefits and any group-specific exceptions.

Pediatric oral care services listed in this policy are covered under the member's medical coverage when the plan includes EHBs; services not listed are not covered under medical coverage and would be the member's financial responsibility unless a prior written agreement exists.
