Covered when ALL conditions are met

Facility charges for dental procedures performed outside the dental office are eligible under the medical benefit when ALL of the following are met:

Member has qualifying medical condition: Member has a significant qualifying medical condition documented that requires the procedure be provided at an inpatient/outpatient hospital or ambulatory surgical center.

Examples include heart disease (including congenital defects and prosthetic heart valve requiring strict anticoagulation), blood dyscrasias (including coagulation defects), endocrine disturbances (including brittle diabetes and adrenal insufficiency), neuromuscular disease (including spastic paralysis and muscular dystonias), pulmonary disease (including asthma not safely managed in an office), mental retardation complicated by seizure disorders/cerebral palsy/behavior disorders, documented severe emotional disturbance/behavioral disorders, rampant caries in patients under 48 months (baby bottle syndrome), and extreme pediatric apprehension with documented unsuccessful office sedation attempts.

Preauthorization is required for BlueCHiP for Medicare and is recommended for Commercial products when requesting facility-based dental care outside the dental office.

Dentist may request preauthorization.

Benefit entitlement and billing: Facility-specific charges (e.g., operating room, anesthesia, medical consults) are eligible for coverage under the member's medical benefit only when the member has applicable medical/dental benefits and the above criteria are met.

Coverage for the dental professional fees is provided through the dental benefit; if the member has no dental coverage, dental service payments are the member's responsibility. Benefits vary by group/contract — refer to the Evidence of Coverage or Subscriber Agreement.

Facility-specific charges (for example, operating room, anesthesia, and medical consults) for dental procedures performed outside the dental office may be billed to the member's medical benefit only when the policy criteria are met and the member has applicable medical/dental benefits. Dental professional fees for services rendered by an oral surgeon or dentist are covered under the member's dental benefit; if the member does not have dental coverage, dental service fees are the member's responsibility. Benefits may vary by group or contract; refer to the Evidence of Coverage or Subscriber Agreement for member-specific entitlements.

If services are determined to be not medically necessary (or are medically necessary but are non-covered benefits under the member's plan), the provider may not charge the member for those services unless the provider has informed the member and obtained the member's written agreement to self-pay in advance. Verify member benefits and obtain any required preauthorization before performing facility-based dental procedures to avoid unexpected member liability.

Examples of qualifying medical conditions include, but are not limited to: heart disease (including congenital defects or prosthetic heart valve requiring strict anticoagulation); blood dyscrasias/coagulation defects; brittle diabetes or adrenal insufficiency; neuromuscular disease (e.g., spastic paralysis, muscular dystrophy); pulmonary disease (e.g., uncontrolled asthma); severe developmental or behavioral disorders with risk of seizure or inability to cooperate; documented severe emotional disturbance; rampant caries in very young children (baby bottle syndrome); and extreme apprehension in children with documented unsuccessful office treatment attempts with sedation. Providers must include relevant documentation when requesting preauthorization.

Background

Some patients have significant medical conditions that make delivering dental care in a standard dental office unsafe or impractical; in these cases, the procedure may appropriately be performed in a hospital inpatient/outpatient setting or an ambulatory surgical center. Examples of qualifying conditions (illustrative, not exhaustive) include cardiovascular disease with anticoagulation needs, blood dyscrasias, brittle diabetes, severe pulmonary disease, neuromuscular disorders, behavioral disorders with seizure history, rampant infant caries, or extreme pediatric apprehension after failed office sedation—documentation of the qualifying medical condition is required to request facility-based care and related charges under the medical benefit.
