BNP measurement may be considered reasonable and necessary when used in conjunction with other medical data (history, physical exam, laboratory studies, chest x-ray, ECG). Appropriate clinical contexts include: distinguishing cardiac from non-cardiac causes of acute dyspnea; distinguishing decompensated CHF from COPD exacerbation in symptomatic patients; guiding GDMT dosing in select euvolemic patients in structured HF programs; establishing prognosis or disease severity in chronic CHF; and aiding therapeutic decisions in suspected amyloidosis. Test must be interpreted with other diagnostics and confounders (eg, ischemia, infarction, renal insufficiency) that affect BNP levels. Note: BNP is covered but not separately reimbursed when used with standard diagnostic evaluation in acute care or ED settings.

Only the ordering physician may obtain and manage prior authorization, appeals, or other authorization-related administrative processes; laboratories or laboratory representatives are prohibited from obtaining or facilitating authorizations or appeals and such actions may result in termination from the BCBSRI provider network and denial of payment for unauthorized services.

Clinical Background and Rationale

Nontraditional lipid and non-lipid biomarkers (for example, lipoprotein(a) [Lp(a)], HDL and LDL subclasses, cystatin C, fibrinogen, Lp-PLA2, B-type natriuretic peptide [BNP], and ceramides) have been proposed to improve individual cardiovascular disease (CVD) risk prediction beyond standard lipid panels. Numerous markers and multi-marker combinations are commercially available and have been studied as alternatives or additions to standard lipid testing for risk stratification and management of CVD. Many individual biomarkers show statistical associations with CVD outcomes in cohort studies and meta-analyses, and some provide incremental predictive information compared with traditional risk factors.

However, the policy notes that for most nontraditional biomarkers and for proprietary multi-marker risk panels the clinical utility has not been established — meaning there is insufficient high-quality evidence that testing changes clinical management in a way that improves health outcomes. While some markers are associated with increased risk and may modestly improve risk prediction, available studies have not demonstrated that using these tests to guide treatment leads to better patient-important outcomes; consequently, many tests or panels are considered not covered or not medically necessary.
