Evidence and coverage summary: Although many nontraditional biomarkers (apo B, apo AI, apo E, Lp(a), BNP, cystatin C, fibrinogen, leptin, Lp-PLA2, HDL/LDL subclasses and particle measures) are associated with increased CVD risk, there is insufficient high-quality evidence that measuring these markers improves clinical management or health outcomes beyond traditional lipid measures.

BNP appropriate uses

BNP-specific clinical uses where testing is considered reasonable and necessary when combined with other data:

BNP appropriate uses: Distinguish decompensated congestive heart failure (CHF) from COPD exacerbation in symptomatic patients; distinguish cardiac versus pulmonary causes of acute dyspnea; establish prognosis or disease severity in chronic CHF to guide therapy; aid therapeutic decision-making in amyloidosis; assist optimal dosing of guideline-directed medical therapy (GDMT) in select clinically euvolemic patients followed in structured heart failure programs.

BNP results must be analyzed in conjunction with history, exam, other diagnostic tests, and may be covered but not separately reimbursed in certain acute care settings.
