Clinical Background

Microvolt T-wave alternans (MTWA) detects beat-to-beat variability in T-wave amplitude using specialized sensors and computer algorithms because routine electrocardiography cannot detect these microvolt fluctuations. The test is performed as a provocative procedure that raises heart rate (typically > 110 beats per minute) and reports an alternans ratio, the number of standard deviations by which the peak T‑wave signal exceeds background noise; an alternans ratio of >= 3 is typically considered positive.

MTWA has been investigated as a noninvasive risk‑stratification tool for sudden cardiac death in patients with prior myocardial infarction, heart failure, cardiomyopathy, or other cardiac disorders and has been proposed to help select candidates for implantable cardioverter‑defibrillator (ICD) therapy. It has also been evaluated for assessment of syncope and for identifying patients who may need further invasive electrophysiology testing.

Prospective multicenter studies and pooled evidence have not demonstrated that routine MTWA testing improves net health outcomes when used to guide ICD placement for primary or secondary prevention. As a result, BCBSRI follows CMS guidance for Medicare Advantage members (MTWA and CPT code 93025 covered under CMS determinations), but for many Commercial products MTWA is considered not medically necessary because the evidence is insufficient to support its routine use to direct ICD implantation.
