Scope: This policy addresses coverage for EKG (Electrocardiogram) services for BlueCHiP for Medicare and Commercial products. Coverage stance: mixed — routine EKG screening is discouraged for asymptomatic, low-risk individuals, while EKGs are covered when there are documented signs, symptoms, or other clinical indications. Providers must document the clinical indication on the claim/medical record; when an EKG (CPT 93000) is filed with a preventive E&M code it is separately reimbursed only if filed with a diagnosis that supports medical necessity.
