Coverage Summary

Scope: This policy addresses coverage for EKG (Electrocardiogram) services for BlueCHiP for Medicare and Commercial products. Coverage stance: mixed — routine EKG screening is discouraged for asymptomatic, low-risk individuals, while EKGs are covered when there are documented signs, symptoms, or other clinical indications. Providers must document the clinical indication on the claim/medical record; when an EKG (CPT 93000) is filed with a preventive E&M code it is separately reimbursed only if filed with a diagnosis that supports medical necessity.

Medical-Necessity Criteria

Covered diagnostic EKG services

EKG services are covered diagnostic tests when the following are met:

ALL of the following

There are documented signs and symptoms or other clinical indications for providing the service
When filed with a preventive E&M code, the EKG is separately reimbursed only when filed with a diagnosis that supports the clinical indication for the test

Not covered / Not routinely recommended

EKG services should not be routinely performed in the following circumstances:

ALL of the following

Routine part of a preventive exam in asymptomatic, low-risk individuals without signs/symptoms, family history, or other clinical indications
Screening asymptomatic adults at low risk for coronary heart disease (per USPSTF and cited guideline recommendations)

Background & Evidence

Guideline summary: Recent guidance from the USPSTF, AAFP, ACC/AHA and AHA advises against routine electrocardiography in asymptomatic, low-risk individuals because there is little evidence that detecting coronary artery disease in this population improves health outcomes and potential harms from false positives and downstream invasive procedures may exceed benefits.

The AHA and ACC guidance note that higher-risk individuals (for example older adults or those with multiple risk factors) may derive benefit from screening based on a calculated risk assessment; screening is not recommended when a patient’s estimated risk is < 10% by a risk assessment tool. The USPSTF issued a D recommendation against screening asymptomatic, low-risk adults with resting or exercise ECG and found insufficient evidence to assess benefits versus harms for intermediate- or high-risk asymptomatic adults.
