Background & Evidence

Homocysteine is a sulfur-containing amino acid that has been evaluated as a potential marker of cardiovascular disease and venous thromboembolism because elevated levels were observed in hereditary homocystinuria and in some epidemiologic studies of CVD and VTE.

Levels of homocysteine inversely correlate with folate, prompting trials of folic acid and B vitamins to lower homocysteine as a modifiable risk factor. Although total plasma homocysteine (the sum of homocysteine and its oxidized forms) can be measured and offered as part of cardiovascular or VTE risk assessment, randomized trials and meta-analyses do not demonstrate that lowering homocysteine improves cardiovascular or VTE outcomes, which undermines the clinical utility of routine testing.
