Three-Day Payment Window Coverage Criteria

Three-day payment window criteria

Covered when the following conditions apply to outpatient services rendered on the date of admission and up to three calendar days preceding the inpatient admission:

Services rendered on the date of admission and up to three calendar days preceding the admission are reviewed under this policy (three-day payment window).

Related versus Unrelated Services

If an admitting hospital renders related diagnostic or related nondiagnostic services within the three days preceding and including the date of the inpatient admission, those services are considered inpatient services and must be included in the inpatient reimbursement.
If outpatient services are unrelated and clinically distinct from the reason for the inpatient admission, the hospital may be reimbursed separately for the unrelated outpatient services; such unrelated non-diagnostic services are not subject to the three-day payment window.

Technical portion of non-diagnostic services (other than excluded services) provided by the hospital or facility within the same health system within the three days preceding or on the date of a member's inpatient admission are considered related to the admission and must be included on the inpatient bill, unless clinically distinct and unrelated.

If non-diagnostic outpatient services are not related to the inpatient admission, the hospital must report condition code 51 on the outpatient claim and retain documentation to support that the services are unrelated.

Append modifier PD to physician preadmission diagnostic and admission-related nondiagnostic services (HCPCS/CPT) subject to the three-day payment window. When PD is appended, reimbursement follows: professional component (PC) only for codes with a TC/PC split; facility rate for codes without a TC/PC split.

Facilities and providers are responsible for submitting appropriate codes and/or modifiers and for providing supporting documentation upon request. The Plan reserves the right to request supporting documentation; failure to adhere to coding and billing policies may impact claims processing and reimbursement.

Services provided during a member's admission that are reimbursed under an all-inclusive payment method should be billed by the facility (not a third party); third-party services provided while the member is admitted are the facility's responsibility and should be included on the inpatient claim.

Exclusions: The following service types are excluded from this policy when performed within the designated period prior to an inpatient admission: ambulance; chemotherapy/cancer-related services; home health agency; hospice; outpatient maintenance renal dialysis; rural health clinic (RHC); skilled nursing facility (SNF); therapies (cardiac, occupational, physical, respiratory); transplants.

The following hospitals or hospital units are excluded or are subject to a one-day payment window instead of the three-day window: cancer hospitals; children's hospitals; long-term care hospitals; inpatient rehabilitation hospitals and units; psychiatric/substance abuse hospitals and units.

Unrelated non-diagnostic servicesIf the preadmission non-diagnostic services are unrelated and clinically distinct from the reason for the member's inpatient admission, the hospital or facility (including provider's practice or clinic) should bill the technical portion of those services separately; such unrelated non-diagnostic services performed prior to the inpatient admission are not subject to the three-day payment window.

Notification requirementHospitals, facilities, physicians' practices, and clinics within the same health system should notify each other of a member's inpatient admission prior to the inpatient stay when services fall within the three-day payment window.

Provider Responsibilities and Billing Requirements

Providers are responsible for submission of accurate documentation of services performed and must submit claims using valid HIPAA‑approved code combinations. The Plan may request supporting documentation and will review claims under code edit protocols and coding software logic; providers should submit additional documentation upon request.

Billing Rule

Facilities and providers must submit appropriate codes and/or modifiers for services rendered within the three‑day payment window; failure to adhere to coding and billing policies may impact claims processing and reimbursement.
