Specific serum tumor markers are reimbursable for identified malignancies and clinical situations; markers or panels not addressed are not reimbursable.

Acute lymphoblastic leukemia (ALL) / Pediatric ALL: Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
Acute myeloid leukemia (AML): Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
B‑cell lymphoma: Beta‑2 microglobulin (B2M) — initial diagnostic evaluation; Serum light chains — Castleman disease only — initial diagnostic evaluation; Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
Bone neoplasms (metastatic and primary): Alkaline phosphatase (ALP) — initial diagnostic evaluation; Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
Breast cancer (metastatic): CA 15‑3 and CA 27.29 — monitoring; Carcinoembryonic antigen (CEA) — monitoring.
Breast implant‑associated anaplastic large cell lymphoma (ALCL): Lactate dehydrogenase (LDH) — initial diagnostic evaluation and staging.
Chronic lymphocytic leukemia / small lymphocytic lymphoma: Beta‑2 microglobulin (B2M) — initial diagnostic evaluation; Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
Colon cancer: Carcinoembryonic antigen (CEA) — initial diagnostic evaluation and post‑treatment surveillance every 3–6 months for 2 years, then every 6 months for a total of 5 years.
Endometrial cancer: CA‑125 — additional diagnostic evaluation and/or surveillance.
Epithelial ovarian / fallopian tube / primary peritoneal cancer: Alpha‑fetoprotein (AFP) and beta‑hCG — initial diagnostic evaluation, during primary chemotherapy, and/or monitoring for complete response; CA‑125 also listed for these cancers.
Extrahepatic cholangiocarcinoma and gallbladder cancer: CA 19‑9 — initial (or postoperative) diagnostic evaluation and/or surveillance; CEA — initial diagnostic evaluation and/or surveillance as indicated.
Hepatocellular carcinoma: AFP — initial diagnostic evaluation and screening and/or surveillance every 3–6 months for 2 years, then every 6 months up to 5 years; CA 19‑9 — initial diagnostic evaluation as indicated.
Intrahepatic cholangiocarcinoma: AFP — initial diagnostic evaluation; CA 19‑9 — initial diagnostic evaluation; CEA — initial diagnostic evaluation.
Kidney cancer: Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
Less common ovarian cancers (including mucinous carcinoma of the ovary and low malignant potential tumors): CA 19‑9, CEA, AFP, CA‑125, inhibin (INHA), and LDH as indicated for initial diagnostic evaluation and monitoring per tumor type and risk/stage.
Medullary carcinoma: Calcitonin (CALCA) — initial diagnostic evaluation, monitoring, and/or surveillance 2–3 months postoperative, then every 6–12 months; CEA — initial diagnostic evaluation and surveillance 2–3 months postoperative, then every 6–12 months.
Melanoma (uveal and cutaneous for metastatic/recurrent disease): ALP and LDH — initial diagnostic evaluation for metastatic or recurrent disease.
Multiple myeloma: Beta‑2 microglobulin (B2M) — initial diagnostic evaluation, staging, and/or follow‑up/surveillance as needed; Serum free light chain — initial diagnostic evaluation and/or surveillance as needed; LDH — initial diagnostic evaluation, staging, and/or surveillance as needed.
Myelodysplastic syndromes and T‑cell / T‑cell lymphomas: Lactate dehydrogenase (LDH) — initial diagnostic evaluation.
Myeloid/lymphoid neoplasms with eosinophilia and tyrosine kinase fusion genes: Tryptase — initial diagnostic evaluation.
Myeloproliferative neoplasms: Lactate dehydrogenase (LDH) — initial diagnostic evaluation and/or monitoring while on and after therapy.
Neuroendocrine and adrenal tumors / MEN2: Calcitonin (CALCA) — initial diagnostic evaluation; CEA — initial diagnostic evaluation.
Occult primary masses (liver, mediastinum, retroperitoneum): AFP — initial/additional diagnostic evaluation; beta‑hCG — initial diagnostic evaluation as indicated; CA‑125 — additional diagnostic evaluation for occult primary adenocarcinoma in those with uterus/ovaries present.
Pancreatic adenocarcinoma and small bowel adenocarcinoma: CA 19‑9 — initial diagnostic evaluation, risk classification, monitoring, and/or surveillance (every 3–6 months for 2 years, then every 6 months for a total of 5 years); CEA — initial diagnostic evaluation and/or surveillance per same schedule for small bowel adenocarcinoma.
Pediatric aggressive mature B‑cell lymphomas and Richter's syndrome: LDH — initial diagnostic evaluation.
Peritoneal mesothelioma: CA‑125 — initial diagnostic evaluation.
Saccrococcygeal teratoma: AFP and beta‑hCG — initial diagnostic evaluation and surveillance up to 3 years.
Small cell lung cancer: LDH — prognostic use.
Systemic light chain amyloidosis: ALP — initial diagnostic evaluation; BNP or NT‑proBNP — initial diagnostic evaluation and staging; Beta‑2 microglobulin — initial diagnostic evaluation; Serum free light chain — initial diagnostic evaluation; LDH — initial diagnostic evaluation; Troponin T — initial diagnostic evaluation and staging.
Systemic mastocytosis: Tryptase — initial diagnostic evaluation, monitoring response to therapy, and/or risk classification.
Testicular cancer (nonseminoma and pure seminoma): AFP, beta‑hCG, and LDH — initial and post diagnostic evaluation, staging, risk classification, post‑treatment follow‑up, and surveillance.
Thymomas and thymic carcinomas / Undiagnosed pelvic mass: AFP and/or beta‑hCG, and inhibin (INHA) expression for assessment of less common ovarian cancers and pregnancy where indicated.
Waldenström macroglobulinemia / lymphoplasmacytic lymphoma: Beta‑2 microglobulin — initial diagnostic evaluation and prognostication at first‑line treatment initiation; Serum free light chain and LDH — initial diagnostic evaluation.
Other / Not covered: All other serum tumor markers not addressed above (alone or in a panel) are not reimbursable; analysis of proteomic patterns in serum for screening and detection of cancer is not reimbursable.

Documentation Required

Documentation and Claims Review — provider responsibilities for accurate documentation and submission of additional materials upon request

Providers are responsible for submission of accurate documentation of services performed. Providers must submit claims using valid HIPAA-approved code sets and appropriate coding guidelines (including UB Editor, AMA CPT, HCPCS, ICD-10-CM/PCS, NDC, DRG, CMS CCI edits, and other CMS guidance). Claims are subject to code edit protocols and review against plan benefits, provider contract terms, medical policies, clinical payment and coding policies, and coding software logic. Upon request, providers should submit any additional documentation to support medical necessity and coding. Failure to provide complete and accurate documentation or requested records may result in claim denial or adjustment.

DefinitionClinical Payment and Coding Policy (CPCP) provides reimbursement and coding guidance for services submitted to BCBSOK and may be interpreted with reasonable discretion by BCBSOK.

PrecedenceIf a conflict exists between a CPCP and a member's plan document, the plan document governs; if a conflict exists between a CPCP and a provider contract, the provider contract governs.

Provider responsibilitiesProviders must submit accurate documentation, use valid HIPAA-approved code sets, and code claims according to industry-standard coding guidelines.
