PSA testing coverage criteria
Reimbursable and non-reimbursable uses of total PSA and percent free PSA are specified below.
ALL of the following
- Total PSA screening may be reimbursable at least once per year for men 50 years or older.
- Annual total PSA screening may be reimbursable for men aged 40-50 years who are at increased risk of developing prostate cancer as determined by a physician.
ALL of the following
- For individuals <76 years with total PSA <1 ng/ml and a normal DRE (if done): repeat screening at 2–4 year intervals.
- For individuals <76 years with total PSA 1–3 ng/ml and a normal DRE (if done): repeat screening at 1–2 year intervals.
