Claim edit and denial criteria

Edits that will deny or validate claims based on modifier presence, diagnosis sequencing, and laterality:

Procedures performed within a 10- or 90-day global period must include appropriate modifier(s) when billed by the same physician; absence or incorrect use can result in denial.

Examples of applicable modifiers include 58, 78, and 79 as described by CMS global surgery guidance.

If a procedure requires an anatomical (laterality) modifier and it is not appended, the claim line will deny.

If a modifier is appended but does not match the appropriate anatomical site, the claim will be denied.

When a procedure has a required modifier reported, the modifier must be submitted in the appropriate field (e.g., box 24D on CMS-1500 or SV101-3 electronically); failure to report required modifiers will deny the line.

Diagnosis coding must follow ICD-10 sequencing and CMS guidelines; certain code types cannot be primary/principal or the only diagnosis and will result in denial.

Diagnosis position and pairing rules (ONE of)

Manifestation codes cannot be listed as the primary/principal diagnosis and cannot be the only diagnosis on a claim; such placement will be denied.
Sequela (7th character 'S') codes cannot be listed as the primary/principal diagnosis or be the only diagnosis on a claim; they must be sequenced after the condition causing the sequela.
Secondary diagnosis codes (codes intended only as additional codes) cannot be used as the primary/principal diagnosis and will be denied if submitted as such or as the only diagnosis.
External cause codes (ICD-10 'V-Y' range) are intended as supplemental codes and cannot be the only diagnosis; claims with only an external cause code will be denied.

ICD-10 Excludes1 (mutually exclusive) code pairs cannot be reported together or in incorrect positions; such combinations will be edited/denied.

Specified chemotherapy administration procedure codes require Z51.11 or Z51.12 as the primary/principal diagnosis; when the encounter is solely for chemotherapy two diagnosis codes are required.

Evaluation and management (E/M) services reported with only a Z00–Z99 diagnosis code (and a preventive medicine service performed the same date) will be denied.

Laterality edits (ONE of)

Laterality Modifier to Diagnosis Mismatch: the RT/LT/50 procedure modifier must correlate with the lateral diagnosis submitted; mismatches will be denied.
Laterality Diagnosis to Diagnosis Mismatch: conflicting lateral diagnoses on the same line (e.g., left and unspecified for the same site) will be edited/denied.
