Providers must submit complete and accurate documentation supporting services rendered and billed. BCBSOK may request supporting documentation at any time to verify medical necessity, coding, or reimbursement. Providers are expected to code claims using valid HIPAA code sets and industry-standard coding guidelines (for example, CPT, HCPCS, ICD-10-CM/PCS, NDC, DRG rules, CMS NCCI edits, and other applicable guidance). Claims are subject to claim review, code edit protocols, and any applicable benefit, contract, or policy terms.

Providers are responsible for submission of accurate documentation of services performed.
Upon request, providers must submit additional supporting documentation to adjudicate a claim or modifier usage.
Failure to adhere to coding and billing policies may affect claims processing and reimbursement.
Claims are subject to plan terms, provider contract language, clinical payment and coding policies, and automated coding/software logic.
When submitting Modifier 22 (Increased Procedural Services): append the modifier only to indicate substantially greater work than usual; use represents the provider’s attestation that work was substantially greater; appending Modifier 22 does not guarantee additional reimbursement and BCBSOK may request documentation to support the increased reimbursement consideration.
