Billing rules and coverage criteria for panels and repeat testing

When all individual tests that comprise a recognized CPT panel have been performed, report the panel CPT code rather than billing each component separately.

Tests performed in addition to those specifically indicated for a particular panel should be reported separately from the panel code.

Do not report two or more panel codes that include the same constituent tests from the same patient collection; use the panel that includes the greatest number of tests when overlaps occur and bill remaining tests individually.

Provider-defined (custom) panels are not recognized as coded panels; report individual component codes accurately.

The Plan reserves the right to bundle individual codes into the appropriate panel code for reimbursement and may re-bundle when documentation indicates a panel was performed.

Do not bill certain Obstetric panel CPT codes (e.g., 80055 or 80081) when syphilis screening is conducted using a treponemal antibody approach (CPT 86780); in that case report the individual tests performed in conjunction with CPT 86780.

Append modifier 91 for repeat testing required for treatment when tests are needed at different periods throughout the day; do not append modifier 91 for reruns due to specimen, equipment, or other issues where only the one-time reportable result was required.

Claims may be denied for failure to append modifier 91 or if repeat testing does not meet standard guidelines.

Laboratory testing is regulated under CLIA; append modifier QW when a test is CLIA waived.

Panel vs individual reporting

When to report a panel codeReport the panel CPT code when all individual tests that comprise the recognized panel have been performed; do not bill the panel separately if component tests are billed individually.

Claims Submission, Documentation, and Bundling

Documentation Required

Submit claims with valid HIPAA codes and supporting documentation

Providers must submit claims using valid HIPAA-approved code sets and accurate documentation; be prepared to provide supporting documentation upon request. Claims are subject to code edit protocols and claim review including benefit coverage, provider contract language, medical policies, clinical payment and coding policies, and coding software logic. Upon request, submit any additional documentation to support the claim.

Use HIPAA-approved code sets (e.g., CPT, HCPCS, ICD-10) and industry-standard coding guidelines.
Ensure orders are member-specific, signed and dated by the ordering health care professional, and include rationale/need.
Be prepared to furnish supporting documentation when requested by the Plan or claim reviewers.

Billing Rule

Plan may bundle individual codes into panel codes when panel was performed

The Plan may bundle individual test codes into the appropriate panel code when documentation indicates a panel was ordered and performed or when claim review/software logic identifies component tests that comprise a recognized panel.

If all individual tests that comprise a recognized CPT panel were performed, the Plan may re-bundle component codes into the panel code for reimbursement.
When documentation shows a panel was ordered and performed but the claim lists individual components, claim reviewers may bundle to the panel code.

Defined Terms

Provider-defined (custom) panel

DefinitionProvider-defined (custom) panel: a custom panel created by a provider is not recognized as a coded panel.

Billing requirementWhen a custom panel is used, the codes for the individual tests in the custom panel must be reported accurately and appropriately.

Plan authorityThe Plan reserves the right to bundle individual codes that belong to a panel; claims may be rebundled when documentation shows a panel was ordered and performed.

CLIA/QW

Regulatory basisLaboratory testing is regulated under the Clinical Laboratory Improvement Amendments (CLIA).

Use of QW modifierIf a test is CLIA waived, providers should append modifier QW.
