Reimbursement criteria and exclusions

Reimbursement for discarded amounts is allowed only under specific conditions and excluded in multiple scenarios.

- Discarded amount must be from a single-use vial or single-use package
- Drug or biological is administered to the member appropriately for the member's medical condition and the unused portion is discarded

- The discarded amount must be billed on a separate line than the administered amount with the correct CPT/HCPCS code, units, and JW modifier for all non-inpatient places of service
- The actual dosage must be reported with the correct CPT/HCPCS code and the correct units of service; units billed must correspond with the smallest dose or vial available and be billed in multiples of the dosage specified in the CPT/HCPCS description (round up to the next highest unit if necessary)
- National Drug Codes (NDC) must be included with the CPT/HCPCS code when billing for drugs to receive NDC-based reimbursement; NDC units billed should correspond to the CPT/HCPCS units billed
ALL of the following
- The dose administered, discarded amount, exact date and time of administration and reason for wastage is clearly and acceptably documented in the medical record
- The discarded drugs or biologicals are not administered to another member/patient

Exclusions (any one applies — not reimbursable)

Discarded amounts from multi-use vials and multi-use packages are not reimbursable
Whole vial or package waste will not be reimbursed in most circumstances; limited reimbursable scenarios include (but are not limited to) IUD insertion billed on same date of service, member death/hospitalization/incapacitation after shipment (home delivery only), replacement after disasters, or theft from member
No reimbursement when member misses appointment or changes mind after preparation; provider or shipper mishandling, damage, or improper reconstitution; drug stored outside required conditions; shipping damage; theft from provider or shipper; or billing for manufacturer overfill
Certain products that fail to deliver after application (e.g., Neulasta OnPro J2506 applied but fails to deliver) are not reimbursable
The JW modifier is not used on claims for hospital inpatient admissions

Units and billing rules for drugs

Billing multiple unitsBill units in multiples of the dosage specified in the full CPT/HCPCS description; if the administered amount is not a multiple, round up to the next highest unit.

Rounding ruleWhen amount administered is not a multiple of the CPT/HCPCS code, round to the next highest unit in the CPT/HCPCS description for that code.

Append the JW modifier on a separate claim line for the discarded amount from single‑use vials/packages for all non‑inpatient places of service. Document the dose administered, discarded amount, exact date and time of administration, and reason for wastage in the member's medical record. The discarded amount must be billed on a separate line with the correct CPT/HCPCS code, units, and the JW modifier. The JW modifier should only be applied to the amount that is discarded. The discarded drugs or biologicals must not be administered to another patient. The plan will reimburse the administered amount and the discarded amount up to the vial/package label amount when these documentation and billing requirements are met.

Denial Risk

Denial risk and reviews

Claims that do not adhere to coding and billing guidelines — including improper JW modifier use or insufficient documentation — may be denied or have payment reassigned. Whole vials or packages billed as waste are generally not reimbursed except in limited, documented scenarios (for example: IUD insertion billed same date, member death/hospitalization after shipment for home delivery, replacement after disaster, theft from member). Wasted/discarded drug and biological claims are evaluated case‑by‑case and BCBSOK may request supporting documentation; reimbursements are determined after review of applicable medical and billing documentation. Refer to the provider section of the BCBSOK website and applicable provider contract for current billing guidelines.
