Reimbursement criteria for iron-related laboratory tests

Reimbursable and nonreimbursable laboratory tests related to iron homeostasis are specified as follows.

ALL of the following

Reimbursable for
- a. Evaluation of an individual with abnormal hemoglobin and/or hematocrit levels.
- b. Evaluation and monitoring of iron overload disorders.
- c. Individuals with symptoms of hemochromatosis (See Note 1).
- d. Individuals with first-degree relatives (See Note 2) with confirmed hereditary hemochromatosis (HH).
- e. Evaluation of individuals with liver disease.
- f. Evaluation of hemophagocytic lymphohistiocytosis (HLH) and Still Disease.
- g. Males with secondary hypogonadism.
- h. Monitoring at a frequency of every 1 to 3 months for: (i) patients with chronic kidney disease who are receiving or being considered for receiving treatment for anemia; (ii) individuals on iron therapy.

ALL of the following

Reimbursable for
- a. Evaluation of iron overload in individuals with symptoms of hemochromatosis (See Note 1).
- b. Evaluation of iron overload in individuals with first-degree relatives (See Note 2) with confirmed hereditary hemochromatosis (HH).
