Coverage criteria for iron-related laboratory tests

Reimbursable uses and non-reimbursable tests as specified by BCBSOK:

ANY of the following

ALL of the following
- Evaluation of an individual with abnormal hemoglobin and/or hematocrit levels.
- Evaluation and monitoring of iron overload disorders.
- Individuals with first-degree relatives with confirmed hereditary hemochromatosis.
- Individuals with symptoms of hemochromatosis.
- Evaluation of individuals with liver disease.
- Evaluation for hemophagocytic lymphohistiocytosis (HLH) and Still Disease.
- Males with secondary hypogonadism (frequency: every 1 to 3 months).
- Evaluation and monitoring in chronic kidney disease for anemia (patients receiving or being considered for treatment for anemia).
- Individuals on iron therapy (evaluation and monitoring).

ANY of the following

ALL of the following
- Evaluation of iron overload in individuals with symptoms of hemochromatosis.
- Confirmed hereditary hemochromatosis.
- Evaluation of iron deficiency anemia.

Ferritin or transferrin measurement for asymptomatic screening is not reimbursable.

In asymptomatic individuals, use as a screening test is not reimbursable.

Serum hepcidin testing (including immunoassays) is not reimbursable.

Use of GlycA testing to measure or monitor transferrin or other glycosylated proteins is not reimbursable.

Carbohydrate-deficient transferrin is out of scope for this policy.

Providers must submit accurate documentation and code claims using valid HIPAA-approved code sets; claims are subject to claim review and providers may be asked to provide additional documentation upon request. Claims are also subject to plan documents, provider contract terms, coding edits and clinical payment and coding policies.

Submit claims using valid HIPAA-approved code sets (CPT, HCPCS, ICD-10, NDC, DRG, etc.).
Code appropriately per industry-standard coding guidelines and edits (UB Editor, AMA CPT, CMS NCCI/CCI rules).
Be prepared to provide additional documentation upon request to support the claim.
Claims are subject to plan documents and provider contract terms which govern in case of conflict.
