Coverage criteria and restrictions

Reimbursement is conditional on correct coding, required documentation, and medical necessity as defined in MED207.154. Certain uses and order types are excluded from reimbursement.

Claims must contain appropriate CPT/HCPCS codes and only services performed by the billing provider may be billed.

Presumptive CPT: 80305-80307; Definitive HCPCS: G0480-G0483, G0659.

Medical record documentation must support the test ordered for the patient and include relevant history, exam, diagnostic results, and a diagnosis consistent with testing.

The ordering clinician must indicate each specific drug or drug class being tested in a signed, dated written order; reference to a standard order or 'custom panel' is not acceptable.

Requisition must include patient identity, ordering provider identity and legible dated signature, collection site, sample type, collection and receipt dates/times, and collector identity.

Laboratory results must identify the testing entity (including name, address, and CLIA number), patient identifiers, ordering provider name and NPI, collection/receive/report dates, facility (if applicable), and complete test results including validity testing.

Definitive quantitative testing should follow MED207.154 and presumptive results should be reviewed and documented by the treating clinician before ordering definitive testing; if definitive testing is ordered without presumptive testing, documentation must support the order.

Exclusions (non-covered uses)

State- or federally-mandated forensic testing (e.g., court-ordered drug screening, forensic examinations) is excluded from this policy's reimbursement criteria.
Non-forensic employment or commercial driver's license (CDL) testing is excluded from this policy's reimbursement criteria.

Non-reimbursable order types and restrictions

Custom panels are not reimbursable; panel testing is restricted to panels published in the current CPT manual.
Reflex (automatic) testing, standing orders, routine screenings without individualized patient assessment, and unspecified 'custom' panels are not reimbursable.

Reimbursement is subject to member benefit/eligibility, applicable medical policy (see MED207.154), correct CPT/HCPCS coding, and documentation review; complying with documentation requirements does not guarantee payment.

Claim Submission and Documentation Requirements

Providers must submit claims with the appropriate CPT/HCPCS codes and maintain complete medical record documentation supporting the drug test ordered. Reimbursement for presumptive testing will be considered for CPT codes 80305, 80306, 80307; reimbursement for definitive testing will be considered for HCPCS codes G0480, G0481, G0482, G0483, G0659. A provider may only bill for services the provider performs (pass-through billing for services performed by a third party is not permitted.

Submit correct CPT/HCPCS codes on claim (e.g., CPT 80305, 80306, 80307; HCPCS G0480, G0481, G0482, G0483, G0659).
Maintain medical record documentation including a written order that specifies each drug or drug class tested (reference to a standard order, 'custom panel' or 'reflex' testing is not acceptable).
Documentation must include relevant history, physical exam, diagnostic test/procedure results, and a diagnosis consistent with findings.
Requisition must include: patient name and DOB; ordering provider name, credentials, and NPI (preferred); legible/electronic signature with date (physician signature stamp or 'signature on file' not acceptable); facility/location of collection; specimen type; date/time of collection; identity of collector; date/time specimen received by lab.
Verify member benefit and eligibility and that the test is covered under applicable medical policy.
Do not bill for state-, federally-regulated, or legally mandated forensic testing if excluded by policy; employment/CDL testing is not addressed by this policy.
